Yeah, this is all stuff I had in mind; I would say that various parts of the Midwest are still operational, though the US South took a disproportionate hit from the Soviets and the PRC piled on Cali because of the density of bases with combat forces in those regions. 

Most of the nukes are in the midwest, but the US forces aren't about to drop that on their own soil, though some extremists might be willing to try to use them as a terrorist device in Soviet-occupied areas. It's a tense sort of situation, since patriot groups are unruly as hell and probably some of the more prominent organizations running the guerrilla resistance. 

I'd say that what US forces there are would be tattered as hell though -- they're relying on guerrillas to keep the Soviets pinned in occupied areas.
