In this kind of setting, it will be significantly harder to create discrete factions and major conflicts, as well as some sort of plotline, compared to say, a millitary conflict, or actualy anything else with a set goal. Fighting merely to amuse some omnipotent being or for one's own gain, wether it be loot or glory, doesn't provide any sort of ideological prupose to fight, which i regard as one of the most major obstacles in creating the aformentioned roleplay elements.
