You're right, I have never really formally addressed this. 

Inter-planar travel, as you know, happens via the nexus. Every plane has access points to the nexus in the form of portals. How many in your plane is up to you, but it truly doesn't really matter too much. These portals are usually in fairly central locations, but again, that doesn't really matter. The nexus and inter-planar travel is [b]NOT[/b] a 'game mechanic', it's a story device. Remember, imagine you're writing chapters of a book. Travel times aren't a thing because that's boring, but that's how travel works for those who wish to be in on the fluffy canon of it all.
