Wait - If you get your first perk at level 4. Then it looks like:

Level Four: 1 Perk
Level Five: 2 Perks
Level Six: 3 Perks
Level Seven: 4 Perks
Level Eight: 5 Perks 
Level Nine: 6 Perks

Edit: Also there are six perk decisions in the first page.
Double Edit: Plus the Black King has six perks.
