Will we decide the theme of our side-effects all together (Mental, physical, etc.) or does it change from player to player?

Personally, I'd like it if we made our own side-effects and PM them to you for approval.

Also, a mix of side-effect types seem good. It leaves space for joke characters, but also has room for serious guys as well.
