Are we allowed to make references toward the fact that the doctor has a name similar to the Russian President Vladimir Putin? Or is this a separate society where dropping that name in the vulgar-common-tongue wouldn't strike a nerve on anyone?

If the Russian President is still Vladimir Putin then my character is going to go insane after meeting the doctor. I mean, further down the never-ending spiral staircase, that is. She'll probably get lost trying to figure it out.
