She's actually more proficient and practiced with her medical techniques than her Hiden ones at this point. She's capable of doing them, but any of her Binds or Imitations have a much shorter longevity to them because she isn't that practiced with them, nor did she have a lot of time to do so during her tenure as a medical shinobi.

I had designed her to be able to use Scalpel offensively in instances when she didn't have to move extensively. She couldn't run at someone with it, but I had imagined if someone charged her, she could move her hands and feet short distances to use it to actually cut someone. She would always lack the control on a moving target to actually cut anything lethal. At most, offensively, I'd imagine she could cause some damage to muscles on an opponent so that utilizing that muscle would cause damage if they continued to use it without getting medical attention (that and hurting a whole hell of a lot). She would lack the ability to precisely cut tendons or anything along that line.

In context to the cannon, she is only two years younger than Kabuto and has a seriously compacted jutsu list to him (even to Sakura, who she would be a senior in age and rank). I'd say she's intelligent enough to have learned the jutsu she has and is proficient enough in most of them to actually make use of techniques Scalpel, even while moving small distances. She didn't spend the time to learn abilities outside of ones for surgery, nor did she learn the Chakra Enhanced Strength. She'd also have a lot more practice with Scalpels than either of them would from having actually been using it extensively for a long time during a conflict.
