Does he necessarily have to literally be clinically insane? Why not have him be just a guy who believes in Boethian/Nietzschean philosophy? Respect none but yourself, legitimize no royal with your hatred of him, only disregard them as what they are- a shape of metal and the weakling hiding under it. You read that reddit link I sent you?
