Speaking as someone who barely watches anime and is among the least worldly people you'll see, I've seen worse. Way worse. The cheery music and kittens actually dulled the shock value more. It feels like it's trying too hard, it quiets the actual audio of people getting massacred, and I've seen that a lot in all sorts of media, so I'm used to it. There's a pretty low level of gore detail, just a lot of blood (like when you shoot a video game model, and all you see is 2D splatters). To the point it's cartoony, almost comedic. It's also not sexually or psychologically fucked up in any way that other anime/Japanese shiz can excel at. The worst, usually, is a story that seems to praise and have a creepy, obsessive enjoyment of destroying people in various morbid ways, like the creator actually wants to do it or does it. An example of something creepier was something I saw from an old set of Kagamine's, actually. ;] The guy with white goop. I didn't even know what was going on, there.
