I'm guessing you've never seen Blade. I'll take out all the super vampire bits but what your looking at is a Reaper. The only way you can tell a reaper is a reaper is when it opens its mouth, which is that monstrosity you saw in the picture. Otherwise the only signs are complete baldness, sickly paleness, and a scar like split from lip to the bottom of the throat.
