[/quote] It's based mostly on logic for me and that seems to work out. If your opponent is swinging a sword at you and you know your character couldn't dodge or block it, at that point you accept the hit and realistic damage that would ensure. If you don't know if your character could realistically have time to dodge then you could discuss it with your opponent. How fast was the swing? How fast is your character? Did the attack surprise them so they wouldn't have enough time to react? etc. It's up to you if you want them to get killed off sooner than it would have to logically happen. For example, if you think you could possibly squeeze out a dodge, you can still say they didn't for some reason.
