There are a few exceptions about the weapon fluff. Like Avitus' Heavy Bolter, which he used to kill about three hundred or something Guardsmen with during the events of Soulstorm. And Godsplitter, which was given to Gabriel Angelos during the first game.

But most others are pretty silly, like what you just said. I remember a power axe called the Fang of Fenris or something, a Salamander made axe, supposed to be for the Space Wolves. But the Wolves denied it and it somehow ended up in the hands of the Blood Ravens.
Or that Bolter that was used by an Adeptus Custodes during the Horus Heresy.
