[@Dragoknighte], I assumed it had back rooms - of which are likely storerooms given what you said, as well as a connecting hallway. The general idea was to have "The Red" check every single room in his path until he ran out of doors or found the gnome, alternatively he would also continue until someone tried to stop him which could go any number of ways.

Based on what you said, the best image and fit would have been in a hall between the front, the store rooms (with people likely in them, waiting or working) and the operating room. Which sort of fits what Fated said as the closure to the hole.
