Actually fun fact, according to the comic spider man never stopped mutating. There was one issue he turned completely into a hybrid spider thing and the Punisher had to come in and help tun him back. You could say he never stopped transforming. 

Though as far as transformations go, that's a rather poor example of one overall. A werebeast, or heck even sailor moon would be better examples.
