You already have it all figured out <3 I could see a potential subplot building where the Yoko side of him grows weary of being the lapdog of Koenma and tackling demons far beneath him. I'll need to refresh my memory of how Yoko and Kurama both work. Didn't Yoko inhabit the body of a woman's baby that wouldn't survive without him as a form of thanks for that woman freeing him from a trap or something like that? 

If Botan's living with him and always around him, the rumors can't be helped, to the point Kurama can only deny them if asked face to face, but of course no one would believe him. Kurama's the popular guy based on looks and intelligence (not to mention strength no one can comprehend there), so for Botan to just show up out of nowhere and assert her position as his secret girlfriend (to them) may look like some form of challenge. With the scenario you named with Botan returning a little worn out and Kurama short of breath, that just increases the rumors even further. As far as Koenma is concerned, it'll be the perfect cover for them to be together for cases so much. He then asks them to pretend, and you know what happens next XP
