[@MetalLover]Since many people mentioned mutants, what kind of "fantastic" elements are allowed?
Even in Fallout the mutants are usually product of genetic experiments and the FEV virus.
Also didn't the nations used fusion nukes that are almost completely void of any fallout? Granted,  a destruction of this scale I bet there are plenty of other hazards.
