They are free to use like the medicine cat in Cliffclan. Medicine cat apprentices don't always get a choice in being picked. Its a sign from Starclan or pushed on them due to injury or not being able to be a warrior. Like Jayfeather being blind. Or Cinderpelt's leg injury.
