As for the vaults in the north, you can make one like a Vault 101 clone if you want and trade out the Pitt for Ronto. Use one of the vaults I listed in the CS area that is allied with the Enclave, not under their control. I can see Vault Tec being [s]Stupid[/s] Insane enough to make another vault that was never supposed to open but did anyway for some magical reason, or it was a control vault and it turned into a town that she happened to be born in.

Oh, and Ronto was a city that was mentioned in the pitt DLC. A lot of speculation said that it was actually what became of toronto. Here, I'd say it's the Pitt, but with a higher population.

I'm absolutely sure that she can't come from the US however as it's literally a huge warzone between the Legion, the Protectron Army, NCR, Raiders, and every other living, breathing thing.
