It tripped me up, first post, too. But after the chunk of greater understanding of the system with which Lady A insists we use for combat in her RP it was a rather easy transition, what with reading her instructions. Just to be certain, I used the same safeguard as she insisted you attempt: Looking and seeing what everyone else does with their headers. 

Now, seeing as she just now gave a warning (and didn't null your last post, read above) with that chunk of advice, I'd say we're doing ok. Not great, obviously, but you now have the framework necessary to format both combat and basic interaction posts. Use headers. When it calls for (in the header) who you're interacting with, post [i]everyone[/i] with which you interact in your post. In the body AND header, as the enemies have been given a clock-style number assigned based upon initial location, use the numbers. 

It makes the process so much easier for the players to visualize and keep track of the fight. Also, as the GM is responsible for every NPC status and action, it makes it significantly more manageable for her.
