Another thing about the spine thing, if every mission or damaging experience messes him up then that might mean every mission he'll have to go back to the clinic. In my mind, that might become a hassle as it means we need to stop there all the time. The one thing about these loyalty missions is that we can determine [i]when[/i] they happen so maybe the first mission caused enough damage to affect, but not need to go to the clinic. However getting another mission right away and not gaining that R&R might be what seriously fucks up his system forcing that Loyalty Mission to be a critical next one. 

If we need to do his loyalty mission right now, and can't wait, then we can do the loyalty mission first rather than the normal. Though I do recall you wanted to wait at one point and only offered it up when Dervish was on his hiatus and for something to do if we didn't have enough missions. Just my thoughts on the matter btw, nothing to offend you I hope.
