Honestly looking at diagrams of the throat...im not sure that if you were dead set on destroying the vocal folds you'd come from the outside. seems like it'd make more sense to go in through the mouth and destroy them from within windpipe. 

So basically not only did she get mutilated, didnt even get mutilated effectively.
