Regarding age, I think it's fine not having to create another section for it. We can just put the age in brackets next to the birthdate.
The reason why I feel it'd be good to indicate the age clearly is because there are people who are just really bad at Math. Not that I am, but I do know people in real life who are - even in my current college class - who are just hopeless even with simple calculations, yeah.

As for "Also Known As", I assume that you mean the nickname or alias the character has. I guess it can be an optional section, because not all characters would require that. In some Western/European names, the nickname/alias may be included in the full name itself, but with quotation marks around it to indicate that it's not part of their real name.
