In terms of the card, yea leave it blank if your not part of the peerage prior. 

If your gonna start out as a human fill in all feels applicable, so no holy sacrements or using former race until after you become an angel. You can basically make your death a plot element in the rp, so, you choose where oyu want to be when you die.

(all are reincarnated initally as two winged angels, lowest of the low)
