If you look at the character sheet you can see all that fancy stuff right?  To get the fonts and formatting that I have used click "View Raw" and copy and paste that.
Then you can put your form in that format.  I also, think you are good to go so after you sort out the formatting you can drop her off in the character tab.  Welcome to Angel Academy, Olivia!
