One possible idea: if there are still multiple countries (instead of a one-nation Earth, like a lot of sci-fi) then perhaps a few countries don't have CARE. Even the knowledge that CARE is not universal could be chaotic. Imagine if the Exceptionals found out- they might try to escape to the non-CARE nations, like 1800's U.S. slaves fleeing to the north.
(They'd have to find out that they're captive in the first place, granted)
