So, a few things... 

[list][*] Birthday and the age of your character don't add up. If your character is 21 at the beginning of the arc, he would have been born in 1998. Otherwise, with his birthday in 2001 he would be 18 at the start.
[*] Personal style needs to be one paragraph. Talk about what type of clothes your character wears, their hairstyle, things like that.
[*] You can give your character a "code-name" if you want to, but you don't have to.[/list]
