1. It's a shady conspiracy-esque experiment by the company MULTI. They have solutions to all of those problems.

2. Researchers in the game, figuring they were left for dead, find new planets and realms being created after a year hiatus. Game-time is also slower than real time. One real year feels like five years within the game.

3. AI, yeah. AI does go further than the NPCS, tho. MULTI INC. primarily used the game to create an AI by viewing character interactions. After the breakthrough MTech technology, they were able to perfectly create a human consciousness. All of the advanced AI was activated after the incident, creating a perfect intelligence.
