Not really. He's not undead or anything. With his heavy clothing on he looks pretty human, albeit with slightly weird skin, since he is a Homunculus. He does have nanoflesh forearms, hands, calves, and feet, as well as a whole lot of extendable biotech tentacles on his back, but they're usually inconspicuous. His inner mouth, teeth, and eyes are also biotech, but they're fairly subtle. If he uses his Bioengineer abilities on himself he can make some changes that stand out more, but yeah, he's usually not that deformed.
