[@Wolvena] It would be more like an earthquake, I believe, to anyone Castiel isn't speaking to.  So, they would feel the rumbling, maybe hear a bit of ringing, and see things falling and breaking.  But Dean's the ones getting his eardrums assaulted, because Cass is speaking to him.
