The +2 mana cost is going to be added to the total note cost and not the final cost, so it will be inflated by the difficulty multiplier. When you use the melody cost calculator, just put 2 under the "cost mod -/+" in the appropriate box and the full mana cost will be under "new total." On basic melodies it usually adds 20 mana to the total cost, which is either unreasonable or fair depending on how expensive the note you're using is.
