I, personally, would rule the Penance Stare as "All Sin Committed" which meanst hat, yes, certain creatures would feel nothing. Galactus kills billions, but he does it out of self preservation, not greed or gluttony or pleasure, he does it because he will die if he doesnt. And he is no more guilty of sin for eating a planet than i am for the millions of Cheeseburgers i've eaten.

Thanos, it could be argued, his people have a different set of criteria for Sin, which is why it doesnt work on him.
