[@TruckKun] Alright so the fixed version is the setup that you needed which has most the info you filled in. So just hit quote, copy that, remove the [ quote ][ /quote ] and refill the blank spots you have in the one I fixed, and use the ones that you just posted to fill those in if that makes any sense.
