[@Necrodancer] Sounds good, just wanted to make sure I understood the intention correctly. The only thing that's missing is the BBC coding of the form for organization purposes (as in, all the bolding and color blocking that helps separate each section for easy identifying).  I'm currently reduced to phone typing only so I can't assist with editing it, but if you need help with that let me know and I'll see if I can enlist someone
