Captains:

One (Shinji), three (Ryan), four (Viole), six (Kurisa), eight (I forget), ten has Ryouta as Lieutenant, eleven has Shiro as third seat, and thirteen (I think Hasayo's character? I'dunno).

I think that's pretty much it, and there's people in all those Squads too I believe, and some without Captains. Pretty much all around.
