Just cause you're a doctor doesn't mean he can't be crazy. Take a look at my character. He's a shipwright who is a pacifist to a fault, but would kill just about anyone who is stupid enough to piss him off. 

A crazy doctor could fight with spears and go a little insane, but still have the sanity to fix people up when they are in need (and s/he didn't cause the wounds her/himself).
I also take it that this is what you have in mind for the dog?
