The eyes are removed from the body for this process unless you're highly against that. It's actually good training as well, since you'll be needing to use your EE to keep homeostasis within your eye-socket if you're kept awake. Otherwise, if you're unconscious, we can simply leave it in and insert it like you're thinking.

Sure. Just attach the lens to the very back of the eye, where it connects to the optical nerve, with the opposite orientation to the lens that focuses light.
