Kiku is left and Kuyu is right. They're builds are the same, they in different poses. The heads are slightly different in same. Kiku does have his bigger eyes and a girlier look about him, Kuyu has smaller, sharper eyes and more aggressive eyebrows.
Though, unintentionally, now that I look at it...Kuyu looks like his hair is a lighter shade than his twin's.

But, I'm mainly speaking about Sakaki back in the old days, before the miracle of cake...or Sakaki business mode. Most days, Sakaki is just going to pat Kiku on the head, then get back to work.

Sakaki threated to cut out an accountant's 'useless eyes', since the guy made a small error in filing a tax return...of course, the mistake was fixed as soon as it was notices, but the guy did quit the very next day and hadn't been seen in the city since. Crazy is part of the business mindset, Kiku is an adorable younger brother type to him.

Though he is a good guy, he tends to pick less honorable people as friends (they take advantage of him). If he doesn't want to/is avoiding answering a question, he'll start speaking in rhymes and/or riddles.
