Thirteen-year old Rochelle "Rachel" Auclair (played by Frederique De Raucourt from "Sucker Punch") has been friendless, ever since she lost her mother and older sister in a house fire. Remaining by herself, Rachel was transferred to a children's foster home, where she was being looked after, and treated for her trauma; her only companions being the handmade dolls her mother made for her tenth birthday, named Blade and twinsies, Peri and Pearl. To escape her demons, and the pain of reality, young Rachel creates an alternate world within her mind that will give her the right amount of security the "real" world couldn't seem to provide her with. This "fantasy" became Rachel's only way of retreat, where it is inhabited by a humanized Blade, Peri, and Pearl (who were her only friends in the real world).

After a few years of her residency, as an adult, Rachel (played by Àstrid Bergès-Frisbey) is released, but still bears the burden of the tragic event, as she tries to adjust on living alone. Having to experience strange visions in her sleep that seem to revolve around the dolls of her childhood, Rachel attends therapy, but due to her heavily-scarred psyche, prompted by the fear of both of her memories and dreams, she was suggested to be transferred to a mental institution that replaced the foster home. Soon, she retreats back into her "fantasy", but, somehow, it's more different than how it was six years ago: From all of her time being "unhappy", her companions are now becoming corrupted and twisted.

Having to claim this during therapy, it was highly recommended by the doctor to use these childhood dolls as therapeutic tools for her "mental instability". However, one evening, Rachel finds her lights flickering, and the door to her room mysteriously opens by itself. As she exits her room, she discovers herself in a hybrid-fusion of the real world and her "fantasy" world, where her humanized friends are seemingly real, now equipped with their own deadly devices...and a dangerous fixation on their "owner". With murder happening around every corner, Rachel's suspicions begin to point to something dark and sinister in the hospital. Unless, it was all just in her head?
