I've never really seen it used well, as like moves people try to use at the end of battles that are apparently strong and don't need a lot of energy(because that's been spent), it comes off as desperation and most don't really include any drawbacks to it, except 'being almost dead'.

Then he's not really transforming in battle, which, is more or less the point of a transformation.

I just prefer not to use them often. Kei'taro being the exception, and DG, so far. But never in a situation where, I'm just about dead, then I use my transformation/ultrapowerfulmove to make a miraculous comback , ala @Fallenreaper's mention of Anime.
Because you should probably always start off as your primary form, hence primary. Why transform into your primary form?

because they almost always end up as asspulls and bullshit. "Oh, you're beating me? One more hit and I die. care to meet my new form? it'll heal me, and make me three times stronger. oh, and much like Frieza... it isn't even my final form.'
