As for the Aeris death, Cloud killed her. The spot Sephy stabbed had no blood even though most other times they show it. And it wouldn't be fatal, only possibly paralyze. So Cloud drowned her paralyzed body XD

I would argue that a complete severing of the spine as well as the puncturing of several major organs is fatal, but to each their own.
