Sukoh had fallen in battle, yes?

Ah.

Death's embrace once more, Sukoh had felt it once before. The harshness of battle followed by a serene silence, then what came next was always beautiful. The great bear's eyes slowly opened and surveyed his new surroundings. Body sore from a battle now in his past, head ringing and fading injuries, Sukoh was in fact dead. With a sniffle from his nose he closed his eyes again. "Oh well." He growled out the words, listening to the slight echo from the vast expanse around him. Even with the worry for the lives he had failed to save, Sukoh had earned a rest. And here there was no chance of any foe attacking him in his slumber, so Sukoh, in all of his massive girth? Rolled over onto his side with a resounding thump. Scratched his belly.

And then?

Went to sleep.

The process for feeling better and then returning would feel a lot quicker without having to be conscious through it. Though where would he go? He can't return to whence he came, but he doesn't know what other world he could freely go to. It was unfair to let those who could not defend themselves go without defense, but Sukoh somehow knew there were going to be drawbacks to death. It's kind of how death works.
Not so quick as you think.
Sukoh's dead, and the fight is over. Snarnorgul wins.
Suddenly his grip loosened on the beast below him, the soundwaves and bloodloss combined were far too much for his body to take. Sukoh's eyes rolled backwards and every single pound of his body went limp and fell on the tanar'ri. The bear's hind legs had been holding up a good bit of his weight since the beginning of the fight, and now they had bent to his foe. Though he had dealt significant damage to his opponent his body was not built to fight this kind of enemy, such raw evil was too great for even this pure of a spirit. His innards scrambled and more than half of his blood spilled to the ground he fell in battle to the snarnorgul.

Before his last breath was cast his jaw slid open and he muttered out a weak cry for someone to stop the monster, anyone. Though not many would hear it, it didn't really matter. Sukoh's lost blood and spinning head made this moment pretty much emptiness and silence, he couldn't even see as his head flopped limply downwards. Letting a good portion of whatever was thrown into his mouth fall out. A flood of blood and scrambled innards were sent careening from his mouth and onto the battlefield where elves and centaurs continued fighting. The awe-inspiring battle of colossi was quite finished at this point, with no fight left in him, and a lot of his own body not left in him. Sukoh dies.
Then Sukoh's dead yo.

The bloodloss would be too great at this point, so I'll write it into my post.
When was it stated that the horns were removed? I don't remember reading that.
Sukoh could not throw Snar, he can't lift himself up anymore. The ass bite is his last effort, which should have worked, but doesn't seem to be doing anything besides giving him a nasty scar on his ass.
Assess the damage?

Sukoh's forelegs are crippled, unable to hold up his own weight and likely going to fall off.
He's lost about 1/5 of his blood.
His stomach is gored, but relatively unharmed due to the immense thickness of his fat. Cosmetically scarred, but fatal no.
His chest has 4 foot deep gouges that are around 3 feet wide, once the horns come out he's going to hemorrhage and die.
The left half of his jaw is shattered in a line upwards with the bottom corner being practically gelatin.
So this just isn't going to be resolved then?

At all?
How would a downward strike even work? That doesn't seem possible.
